Avalanche.org is exactly what I had in mind only for backpackers & hikers not snowboarders or skiers. I was also thinking of publications like "Accidents in North Amercan Mountaineering", the National Speleological Society's newsletter, and "River Safety Reports".
If there is one like this for hikers & backpackers it would be useful to read because it would be able to tell us what risks to avoid.
To quote Laurence Gonzalez in his book, "Deep Survival", reading these reports is helpful because it is the closest we can come to "talking to the dead.", i.e., learning from the mistakes that led to fatalities.
But the lack of such a resource may indicate that backpacking & hiking is a lot safer than mountaineering, skiing, snowboarding, or river sports.
